Learning About Chronic Care Management

Learning the basics of Chronic Care Management can make the decision to start it much easier. This comes down to understanding what it is, how to sign up, and the benefits of the program overall. Once you can see the characteristics of the program overall, it is time to get started! Durham Nephrology is here for you every step of the way.

What is it?

Chronic Care Management is a Medicare program designed for Medicare beneficiaries. The main goal of this program is to offer support tailored to the patient through a quality-driven healthcare approach for patients with two or more chronic conditions. This program brings with it multiple benefits to make patients’ situations easier and more manageable. This frequently is expected to go on for at least 12 months or until the patient passes away.

The American Academy of Family Physicians explains that there are five CPT codes that are used to report CCM services that can be found here. This provides you with an even deeper understanding of the standards that go into the program, and its many services.

How do you get signed up?

You may be wondering how to be a part of this program. The process of signing up is not stressful. It falls into the hands of the patient to decide whether this is a good option for them. Those who are eligible must give consent to enroll in the Chronic Care Management program. This only requires calling (919) 335-6634 to get the process started. If you have any questions about this process please contact our office for more information and assistance.

What are the benefits?

There are numerous benefits for patients participating in the Chronic Care Management program. They stem from time-saving to communication growing opportunities that can all change the way you control your health journey. The first of those stems from the fact that medical professionals are on standby to answer your questions at any time during the day or night. Once a month, patients can expect a check-in with the clinician to assess their current and long-term health needs. This also comes with a personalized health plan that improves the quality of life. Alongside the planning, the program can help with refills, scheduling appointments, and transportation. Each of these aids you in having a smoother overall health journey.

Chronic Care Management may be the solution you’ve been looking for. There is so much to learn and understand about how the program can positively benefit your life. Our team is paired with an excellent resource in ChartSpan that can make this an easy process for you.
